schedule Last revised: 14 February 2025 • bookmark Instrument CONSENT-UG-1.2

1. What you consent to

  • Creation and maintenance of your Client Wellness Diary (CWD) for consultancy purposes.
  • Assigned Comprehensive and/or Holistic assessments, questionnaires, and progress tracking.
  • Secure sharing of approved records with your assigned consultant and supervising practitioners.
  • Storage of encrypted records on Uganda-aligned regional infrastructure.
  • Delivery of notifications about assignments, released reports, and account security events.

2. What THEME is not

THEME Wellness is a culturally grounded consultancy and assessment workspace. THINQ scores, NMD/MD flags, and related outputs are not medical diagnoses, prescriptions, or emergency clinical care. See the Wellness & Medical Disclaimer.

3. Faith-based and Holy Spirit Mode content

Faith-based pathways (including Holy Spirit Mode) are opt-in only. Clients are never required to enable Divine/SVI scoring or faith media without explicit consent.

4. Versioning, withdrawal, and your rights

  • Each consent event records instrument version, date/time, and status (active / withdrawn).
  • You may request withdrawal or data export through Account settings or privacy@themewellness.ug, subject to lawful retention.
  • Guardian consent and age-gating apply where minors are supported.
